The Mission

Our ultimate goal is to provide the highest quality of care to the children and adolescents with behavior issues by treating them with proven therapeutic techniques that will assist with declining negative behaviors that have hindered their everyday life.

Extended Reach provides help to children and adolescents who suffer from unstable homes, conduct disorders, adjustment problems, severe mood swings, school issues, defiance issues, frequent rage outbursts, isolation, attention deficit disorder, depression, and other behavioral problems that may be affecting your troubled youth.

Day Treatment

This program is intended for individuals with moderate to severe behavioral disruptive disorders whose needs have not been effectively met through weekly individual or group therapy sessions. It is also for individuals whose symptoms significantly disrupt daily activities such as work, school, and relationships, which can result in social awkwardness, and other behaviors that would otherwise disturb normal functioning within the child’s life. This program is offered Monday through Friday from 11am to 7pm, and length of stay is generally 2 – 6 months, (dependent upon the behavior needs of the individual served). Each client works individually with a therapist in addition to a Qualified & Associate Professional, so that each person’s unique needs are appropriately addressed and treated.

Extended Reach Services evaluate and treat behavioral, emotional, social and psychiatric difficulties.

We provide treatment for conditions, such as:

  • Anxiety disorders
  • Obsessive-compulsive disorder
  • Post-traumatic stress disorder
  • Depression
  • Op-positional defiant disorder
  • Attention-deficit hyperactivity disorder
  • Substance abuse (alcohol and other drugs)
  • Suicidal thoughts
  • Autism and behavior disorders
  • Pervasive developmental disorders

Each child seen at Extended Reach receives a comprehensive assessment and an individual treatment plan. Treatment plans are developed to meet the needs of children and families. We may also work with other departments, depending on the child’s needs.

Intensive Home Services

Intensive In-Home Services are provided for children/adolescents under the age of 18. This service is available to the community 7 days per week to allow for maximum flexibility needed for effective care. The primary goal of the service is to stabilize youth in the home setting and to decrease the risk of placement outside of the home or to transition youth to home from an out of home placement. For purposes of this service, home is defined as the family residence, and includes a child living with natural parents, relatives, or guardians, or the family residence of the child’s permanent or temporary foster care or pre-adoption family residence.

These services are time-limited interventions (up to 26 weeks) provided typically but not solely in the residence of the child. Any extension of these services must be preauthorized. In-Home services include crisis intervention services, psychiatric and pharmacological services, individual/ family counseling/therapy and communication skills building (e.g. counseling to assist the child and his parents to understand and practice appropriate problem-solving, anger management, and interpersonal interaction, etc.) case management, and 24 hour emergency response. The focus of this time-limited intervention is to:

  • Defuse the current crisis.
  • Provide intensive case management to link the family to other needed community services.
  • Individual, couples and/or family therapy.
  • Parent skills training to increase family’s ability to effectively problem solve and manage behavioral issues. “Family Systems” focused interventions.
  • Monitor and manage the presenting psychiatric and/or addiction symptoms.
  • Services typically range from 2 to 8 hours of intensive therapeutic services per week.

Outpatient/Medication Management

Our outpatient services encompass a variety of services such as… Assessments and Evaluations, Individual and Family Therapy/Counseling, Psychological Testing and Medication Evaluations and Referrals for children and adults.

Before any services can begin, you must have an intake assessment/evaluation to determine the nature and severity of their behavioral and/or emotional difficulties. Extended Reach services can provide this service in the home or office.

This evaluation often results in a mental health or substance abuse diagnosis. Depending on the results of this assessment/evaluation, your insurance company will approve/authorize certain mental health services with the goal of improving the child’s behavior and/or emotional state. While receiving services from our agency, a re-assessment may be necessary if new issues begin to develop or a crisis situation arises.

If you are in need of psychological testing and/or a medication evaluation, your Qualified Professional or Therapist can refer you to our staff psychologist or psychiatrist. These services are provided in an office setting.

Individual and Family Therapy

Extended Reach has the ability to provide individual and family therapy (counseling) in the home or office. For children who are experiencing behavioral problems in the school setting, they may be eligible to receive individual therapy at school through our agency. Individual and family therapy services are provided for children and adults by a professional who has both a Masters Degree in Counseling, Social Work or Psychology and a State License. Therapy sessions usually occur one time per week for one hour (can last up to two hours if needed). Therapy can assist a child and parent in gaining insight into the root cause of behavioral and/or emotional difficulties and can assist in developing solutions to these difficulties with the goal of ultimately changing behavior. Many families find that therapy can reduce their stress levels as they learn to talk through and resolve their issues.

Targeted Case Management (Mental Health)

Each child participating at Extended Reach will work closely with an individual often referred to as a case manager. Your child’s case manager is a mental health professional with either a Bachelor’s or Master’s degree in Psychology, Social Work, or a counseling field.

Case Management connects individuals with mental health and behavioral concerns to the community services they need. Each individual who is referred and authorized to receive services is assigned a Case Manager to coordinate their care. Following a thorough assessment, the Case Manager and the individual will work together to develop a treatment plan that identifies and prioritizes their service needs. The Case Manager will then connect the individual to the appropriate service provider. A successful outcome is achieved when the individual obtains services and supports needed to live independently in the community and no longer requires case management services.
