Targeted Case Management

Multi-racial Hands Holding Each OtherEach child participating at Extended Reach will work closely with an individual often referred to as a case manager. Your child’s case manager is a mental health professional with either a Bachelor’s or Master’s degree in Psychology, Social Work, or a counseling field.

Case Management connects individuals with mental health and behavioral concerns to the community services they need. Each individual who is referred and authorized to receive services is assigned a Case Manager to coordinate their care. Following a thorough assessment, the Case Manager and the individual will work together to develop a treatment plan that identifies and prioritizes their service needs. The Case Manager will then connect the individual to the appropriate service provider. A successful outcome is achieved when the individual obtains services and supports needed to live independently in the community and no longer requires case management services.
